सुचना के अधिकार अधिनियम 2005 के तहत "कूडेदान की सफाई नहीं होने " की जानकारी लेने सम्बन्धी आवेदन पत्र का प्रारूप निचे दिया गया है -

महोदय,
कूड़ेदान का पता / Trash Address :
उपयुर्क्त कूड़ेदान से सम्बंधित निम्नलिखित सूचनाएं उपलब्ध् कराएं:
1. उस डिपो का पता बताएं जहां से लोडर तथा ट्रक इस कूडेदान के लिए भेजे जाते हैं?
2. उन लोडर तथा ट्रक का नंबर बताएं जो इस कूडे़दान से कूड़ा उठाने के लिए नियुक्त हैं?
3. दिनांक .....................से .......................... के बीच डिपो के बीट रजिस्टर में दर्ज इन गाड़ियों के डिपो छोड़ने तथा डिपो में वापस आने के समय का पूरा विवरण उपलब्ध् कराएं?
4. उन सभी कूडे़दानों का पता बताएं, जहां का कूडा उपर्युक्त समय के दौरान इन गाड़ियों ने उठाया?
5. इस दौरान प्रतिदिन इन गाड़ियों द्वारा लगाए गए चक्करों का विवरण दें?
6. इस दौरान प्रत्येक चक्कर में इन गाडियों द्वारा उठाए गए कूडे के वजन का विवरण दें?
7. उपर्युक्त कूड़ादान पिछले ................ दिनों से साफ नहीं किया गया है। इस दौरान इस क्षेत्र के सेनेटरी इंस्पेक्टर द्वारा भेजी गई प्रतिदिन के बैलेंस रिपोर्ट कि प्रति उपलब्ध् कराएं?
8. क्या उपर्युक्त बैलेंस रिपोर्ट में इस बात का उल्लेख है इस दौरान कूडे़दान का कूड़ा नहीं उठाया गया? यदि नहीं, तो सम्बंधित क्षेत्र के सेनेटरी इंस्पेक्टर ने इसका उल्लेख क्यों नहीं किया?
मैं आवेदन फीस के रूप में 10रू अलग से जमा कर रहा /रही हूं।
या
मैं बी.पी.एल. कार्ड धारी हूं इसलिए सभी देय शुल्कों से मुक्त हूं। मेरा बी.पी.एल. कार्ड नं..............है।
यदि मांगी गई सूचना आपके विभाग/कार्यालय से सम्बंधित नहीं हो तो सूचना का अधिकार अधिनियम, 2005 की धारा 6 (3) का संज्ञान लेते हुए मेरा आवेदन सम्बंधित लोक सूचना अधिकारी को पांच दिनों के समयाविध् के अन्तर्गत हस्तान्तरित करें। साथ ही अधिनियम के प्रावधानों के तहत सूचना उपलब्ध् कराते समय प्रथम अपील अधिकारी का नाम व पता अवश्य बतायें।

2. Give the number of loaders and trucks that are employed to pick up the garbage from this dustbin?
3. Date .....................to ..................................... Provide complete details of the time of departure of these trains and their return to the depot as recorded in the beat register of the depot between ?
4. Give the address of all the dustbins where these trains picked up the garbage during the above mentioned time?
5. Give the details of the number of rounds made by these trains daily during this period?
6. Give the weight of the garbage carried by these trains in each round during this period?
7. The above dustbin has not been cleaned since last ................ days. During this, provide a copy of the daily balance report sent by the sanitary inspector of this area?
8. Whether it is mentioned in the above balance report that the dustbin was not picked up during this period? If not, why did the Sanitary Inspector of the concerned area not mention it?
