सुचना के अधिकार अधिनियम 2005 के तहत "स्ट्रीट लाइट" की खराबी से सम्बंधित विभिन्न प्रकार की जानकारी लेने सम्बन्धी आवेदन पत्र का प्रारूप निचे दिया गया है -

महोदय,

निम्नलिखित स्ट्रीट लाइट बहुत दिनों से काम नहीं कर रहीं है:

इसके लिए कई शिकायतें की जा चुकी है् (प्रति संलग्न है) लेकिन इन शिकायतों पर अब तक कोई कार्रवाई नहीं की गई है। इस सम्बंध् में निम्नलिखित सूचनाएं उपलब्ध् कराये:

1. नगर निगम ने इस क्षेत्र की स्ट्रीट लाइटों के रख रखाव का ठेका किसे दिया है? उस ठेके की प्रति दें?

2. नागरिकों द्वारा शिकायत किए जाने के बाद कितने दिनों के अन्दर खराब लाइटों की मरम्मत हो जानी चाहिए? इससे सम्बंधित कॉनट्रेक्ट या यदि कोई आदेश दिया गया हो तो उसकी प्रति दें?

3. यदि लाइटों की मरम्मत उपरोक्त समय सीमा के अन्दर नहीं होती है तो ठेकेदार के खिलाफ क्या कार्रवाई की जाती है? इससे सम्बंधित कॉनट्रेक्ट या यदि कोई आदेश दिया गया हो तो उसकी प्रति दें?

4. किन परिस्थितियों में ठेकेदार के भुगतान में कटौती की जाती है? इससे सम्बंधित कॉनट्रेक्ट या यदि कोई आदेश दिया गया हो तो उसकी प्रति दें?

5. क्या मेरे द्वारा किए गए शिकायतों के सन्दर्भ में ठेकेदार के भुगतान में कटौती की जाएगी? यदि नहीं तो क्यों?

6. यदि हां, तो कितने दिनों के अन्दर नगर निगम ठेकेदार के भुगतान में से कटौती कर लेगी?

7. किन परिस्थितियों में कॉनट्रेक्ट रद्द किया जा सकता है? इससे सम्बंधित कॉनट्रेक्ट या यदि कोई आदेश दिया गया हो तो उसकी प्रति दें?

8. क्या मेरे द्वारा की गई शिकायतों के सन्दर्भ में कॉनट्रेक्ट रद्द किया जा सकता है?

9. यदि हां, तो कितने दिनों के अन्दर नगर निगम कॉनट्रेक्ट रद्द कर देगा?

10. अगर ठेकेदार अपना काम सही तरीके नहीं करता है, तो नगर निगम के पास कौन कौन सी शक्तियां हैं जिनका प्रयोग करके वो ठेकेदार को सही तरीके से काम करने के लिए बाध्य कर सकता है?

मैं आवेदन फीस के रूप में 10रू अलग से जमा कर रहा /रही हूं।

या

मैं बी.पी.एल. कार्ड धारी हूं इसलिए सभी देय शुल्कों से मुक्त हूं। मेरा बी.पी.एल. कार्ड नं..............है।

यदि मांगी गई सूचना आपके विभाग/कार्यालय से सम्बंधित नहीं हो तो सूचना का अधिकार अधिनियम, 2005 की धारा 6 (3) का संज्ञान लेते हुए मेरा आवेदन सम्बंधित लोक सूचना अधिकारी को पांच दिनों के समयाविध् के अन्तर्गत हस्तान्तरित करें। साथ ही अधिनियम के प्रावधानों के तहत सूचना उपलब्ध् कराते समय प्रथम अपील अधिकारी का नाम व पता अवश्य बतायें।

Sir,

The following street lights are not working for a long time:

Many complaints have been made for this (copy is attached) but no action has been taken on these complaints so far. Please provide the following information in this regard:

1. To whom has the Municipal Corporation given the contract for the maintenance of street lights of this area? Give a copy of that contract?

2. Within how many days after a complaint is made by the citizens, the faulty lights should be repaired? Give a copy of the contract related to this or if any order has been placed?

3. If the repair of lights is not done within the above time limit then what action is taken against the contractor? Give a copy of the contract related to this or if any order has been placed?

4. Under what circumstances is the contractor's payment deducted? Give a copy of the contract related to this or if any order has been placed?

5. Whether payment of contractor will be deducted in respect of complaints made by me? If not why?

6. If yes, within how many days the Municipal Corporation will deduct from the payment of the contractor?

7. Under what circumstances can the contract be cancelled? Give a copy of the contract related to this or if any order has been placed?

8. Can the contract be canceled in respect of complaints made by me?

9. If yes, within how many days will the Municipal Corporation cancel the contract?

10. If the contractor does not do his work properly, then what are the powers of the Municipal Corporation by which it can force the contractor to do the work properly?
