मेरा घर नगर निगम की जिस बीट में आता है, उस बीट की सफाई व्यवस्था से सम्बंधित निम्नलिखित सूचनाएं उपलब्ध् कराएं:

1. इस बीट में नियुक्त सभी स्वीपरों तथा सफाई कर्मचारियों/अधिकारियों की सूची उनके नाम तथा संपर्क के पते और नंबर के साथ उपलब्ध् कराएं?

2. इस बीट का ..................... माह की उपस्थिति रजिस्टर की प्रति उपलब्ध् कराएं?

3. इस बीट की ..................... माह का मस्टर रोल की प्रति उपलब्ध् कराएं?

4. इस बीट में नियुक्त सभी स्वीपरों के नाम के साथ जिस सड़क या क्षेत्र की प्रतिदिन सफाई के लिए वे जिम्मेदार हैं, उसका विवरण दें?

5. उन अधिकारियों के नाम, ऑफिस का पता तथा संपर्क के नंबर की सूची दें, जो इस बीट के स्वीपरों के कार्य के निरीक्षण के लिए जिम्मेदार हैं?

मैं आवेदन फीस के रूप में 10रू अलग से जमा कर रहा /रही हूं।

यदि मांगी गई सूचना आपके विभाग/कार्यालय से सम्बंधित नहीं हो तो सूचना का अधिकार अधिनियम, 2005 की धारा 6 (3) का संज्ञान लेते हुए मेरा आवेदन सम्बंधित लोक सूचना अधिकारी को पांच दिनों के समयाविध् के अन्तर्गत हस्तान्तरित करें। साथ ही अधिनियम के प्रावधानों के तहत सूचना उपलब्ध् कराते समय प्रथम अपील अधिकारी का नाम व पता अवश्य बतायें।
