सूचना के अधिकार के तहत "सड़क की खुदाई का विवरण" लेने हेतु आवेंदन पत्र (Application form for taking "Details of road excavation" under Right to Information) :

 सूचना के अधिकार के तहत "सड़क की खुदाई का विवरण" लेने हेतु आवेंदन पत्र का प्रारूप निचे दिया गया है -

महोदय,

................. क्षेत्र में दिनांक ......................से दिनांक .................के दौरान सड़क काटने, और उससे बने खड्डों को पुन: भरने के लिए विभिन्न एजेंसीयों द्वारा किए गए सभी कार्यों से सम्बंधित सूचनाएं उपलब्ध् कराएं :

1. उन एजेंसियों की सूची जिन्होंने इस दौरान सड़कों में कटिंग या खुदाई का कार्य किया। सड़क काटने की अविध् बताएं?

2. इन एजेंसीयों द्वारा सड़क काटने के लिए कितनी राशि जमा की गई तथा यह राशि कब जमा की गई?

3. उन सभी स्केचों की प्रति जिसमें उन जगहों को दर्शाया गया हो जहां एजेंसीयों को सड़क काटने के लिए अनुमति दी गई।

4. क्या सभी एजेंसीयों ने स्वीकृत स्थानों पर हीं सड़क काटी या इसमें परिवर्तन भी हुआ? यदि हुआ तो इसकी जानकारी दें।

5. आपके विभाग द्वारा उपरोक्त जमा की गई राशि का उपयोग कहां किया गया? उन सभी कार्यों की सूची दें जिसमें इस राशि का उपयोग किया गया, इसमें सभी कार्यों के नाम, जगह का नाम जहां कार्य किया गया है तथा प्रत्यक कार्य के लिए स्वीकृत राशि का पूरा विवरण हो?

मैं आवेदन फीस के रूप में 10रू अलग से जमा कर रहा /रही हूं।

मैं बी.पी.एल. कार्ड धारी हूं इसलिए सभी देय शुल्कों से मुक्त हूं। मेरा बी.पी.एल. कार्ड नं..............है।

यदि मांगी गई सूचना आपके विभाग/कार्यालय से सम्बंधित नहीं हो तो सूचना का अधिकार अधिनियम, 2005 की धारा 6 (3) का संज्ञान लेते हुए मेरा आवेदन सम्बंधित लोक सूचना अधिकारी को पांच दिनों के समयाविध् के अन्तर्गत हस्तान्तरित करें। साथ ही अधिनियम के प्रावधानों के तहत सूचना उपलब्ध् कराते समय प्रथम अपील अधिकारी का नाम व पता अवश्य बतायें।

1. List of agencies who did cutting or digging work in the roads during this period. What is the duration of road cutting?

2. How much amount was deposited by these agencies for cutting the road and when was this amount deposited?

3. Copies of all the sketches showing the places where the agencies were allowed to cut the road.

4. Did all the agencies cut the road at the approved places or did it change? If so, please inform about it.

5. Where was the above deposited amount utilized by your department? Give a list of all the works in which this amount was used, including the names of all the works, the name of the place where the work has been done and the full details of the amount sanctioned for each work.
