सूचना के अधिकार के तहत "सड़क की मरम्मत का विवरण" की मांग हेतु आवेदन पत्र" Application for seeking "Details of repair of road" under Right to Information :

सूचना के अधिकार के तहत "सड़क की मरम्मत का विवरण" की मांग हेतु आवेदन पत्र" का प्रारूप निचे दिया गया है -

महोदय,

नीचे सड़कों की एक सूची दी गई है -

(यहां सड़कों का विवरण दे)

उपयुर्क्त सड़कों के सम्बंध् में निम्नलिखित सूचनाएं उपलब्ध् कराएं :

1. दिनांक ............... से .................. के बीच उपरोक्त प्रत्येक सड़क की मरम्मत (थोड़ी बहुत या बड़े पैमाने पर) कितनी बार हुई?

2. यदि कार्य विभाग द्वारा कराया गया, तो प्रत्येक ऐसे कार्य के सम्बंध् में निम्न सूचनाएं उपलब्ध् कराएं :

क. इस कार्य से सम्बंधित स्टॉक रजिस्टर की प्रति,

ख. कार्य से सम्बंधित लेबर रजिस्टर की प्रति,

ग. उन जगहों की वास्तविक स्थिति, जहाँ कार्य किया गया,

घ. कार्य कब हुआ,

ड. कार्य के लिए प्रयोग की गई सामग्री का मिश्रण क्या था?

3. यदि कार्य ठेकेदार द्वारा कराया गया, तो उससे सम्बंधित निम्न सूचनाएं उपलब्ध् कराएं:

क. मेज़रमेंट बुक की प्रति (एबस्ट्रेक्ट व रिकॉर्ड: दोनों ही प्रविष्टियों का विवरण),

ख. स्केच की प्रति,

ग. खर्च के आकलन के विवरण की प्रति,

घ. यदि कांट्रेक्ट में किसी प्रकार की गारंटी की व्यवस्था थी, तो उसके विवरण की प्रति उपलब्ध् कराएं तथा उन स्थितियों का विवरण दें जिसमें उपरोक्त गारंटी व्यवस्था प्रभावी होती है।

ड. उन सहायक तथा कार्यपालक अभियन्ताओं के नाम बताएं, जिन्होंने इन प्रत्येक कार्यों का निरीक्षण किया और भुगतान की स्वीकृति दी। इनके द्वारा कार्य के किस भाग का निरीक्षण किया गया?

च. क्या अब तक कभी गारंटी व्यवस्था का प्रयोग किया गया है?

4.  सूचना का अधिकार अधिनियम, 2005 की धारा 2(जे)(iii) के तहत मैं इन सड़कों के निर्माण में प्रयुक्त सामग्री का विभाग द्वारा प्रमाणित नमूना लेना चाहता हूं। नमूना मेरे द्वारा चयनित स्थान से मेरी उपस्थिति में एकत्र किया जाए और यह सीलबन्द हो तथा विभाग द्वारा यह प्रमाणित किया जाए कि सीलबन्द नमूना कार्य की सामग्री का असली नमूना है। कृपया मुझे दिन, समय तथा स्थान की सूचना दें जब मैं नमूना लेने के लिए आ सकूं।

5. अब इन सड़कों की मरम्मत कब होगी?

मैं आवेदन फीस के रूप में 10रू अलग से जमा कर रहा /रही हूं।

या

मैं बी.पी.एल. कार्ड धारी हूं इसलिए सभी देय शुल्कों से मुक्त हूं। मेरा बी.पी.एल. कार्ड नं..............है।

यदि मांगी गई सूचना आपके विभाग/कार्यालय से सम्बंधित नहीं हो तो सूचना का अधिकार अधिनियम, 2005 की धारा 6 (3) का संज्ञान लेते हुए मेरा आवेदन सम्बंधित लोक सूचना अधिकारी को पांच दिनों के समयाविध् के अन्तर्गत हस्तान्तरित करें। साथ ही अधिनियम के प्रावधानों के तहत सूचना उपलब्ध् कराते समय प्रथम अपील अधिकारी का नाम व पता अवश्य बतायें।

Below is a list of roads -

Provide the following information regarding the above roads:

1. Repairs to each of the above roads (slightly or on a large scale) between the dates ............... to .............. But how many times?

2. If the work is done by the department, provide the following information in respect of each such work:

a. A copy of the stock register relating to this work,

b. copy of the labor register related to the work,

c. the actual location of the places where the work was done,

d. when the work was done,

d. What was the mixture of materials used for the work?

3. If the work is done by the contractor, please provide the following information related to it:

a. Copy of Measurement Book (Abstract and Record: Details of both the entries),

b. copy of the sketch,

c. a copy of the statement of cost estimation,

d. If the contract provided for any guarantee, provide a copy of the particulars thereof and the circumstances in which the above guarantee arrangement is effective.

e. Name the assistant and executive engineers who inspected each of these works and approved the payment. Which part of the work was supervised by them?

f. Has the guarantee system ever been used?

4. Under Section 2(j)(iii) of the Right to Information Act, 2005, I would like to take a certified sample of the material used in the construction of these roads by the Department. The sample should be collected from the place selected by me in my presence and it should be sealed and certified by the department that the sealed sample is the original sample of the material of the work. Please inform me of the day, time and place when I can come to collect the sample.

5. Now when will these roads be repaired?

I am depositing Rs.10 separately as application fee.
